This is 100% ground crew error once again.
OK..THIS IS HAPPENING MORE AND MORE FREQUENTLY AND NEEDS RO STOP BEFORE A SERIOUS INCIDENT OCCURS..RUNWAY INCIDENTS WITH NEAR MISSES AND ABORTED LANDINGS ARE ATC ISSUES..GROUND IPROBLEMS WITH PUSHBACKS AND RAMP TAXIS ARE GROUND PERSONNEL AND CREWS..BETTER TAINING AS TO WHO DOES WHAT,SPACING AND COMMINICATION IS A NECESSITY!
