I don't see how just an iced over pitot tube would cause a crash or the fact that it was iced over leads to the belief of some kind of fault unless the pitot heat failed. I would assume that would be checked as part of the walk around by the pilot or by ground crew and verified by the pilot.
Those sensors do have heat,(Pitot) so Ice formation would not be a factor-assuming that they; (The flight crew), had the heat turned on, also providing there were no breaches in the electrical circut, ie; a bad fuse.
In the event of a pitot blockage the dynamic pressure relative to the static port would drop to zero and show an IAS of 0 kts. If the static port gets blocked then you can over or under reads with elevation changes. Only scenario I can think of is that if the aircraft was on auto throttle and the IAS was dropping it would go to full power and perhaps push past Vne or something. But surely must be some safeguards to that.
