I found this video explanation which includes video of the windscreen preparing to fail. The reason for the failure is included and apparent when you see the video. The cause is something I never knew existed inside these windscreens. It was electrical malfunction.

When I heard that one of the pilots was sucked partially out of the plane, BA 5390 was the first thing I thought of too

First thing I thought of too. Stripped and undersized windscreen screws I believe.

The windscreen on Ba 5390 was new but the bolts where an 1/8 of a inch to small for the aircraft. They weren’t stripped but the mechanic matched them up by eye in a dark hanger and thought they were correct. However the Ba windscreen was screwed in from the outside of the plane as where the Airbus A319 is bolted or screwed in from inside the cabin and air pressure should help it stay in.

You see two things here.One is the probability of an improper instillation of the window.Number two ,is unless the FO was getting out of his seat to go to the loo, why was he with out his seat belt?

The investigation report for BA-5390 incident states that both pilots had released their shoulder harnesses and the Capt. had loosened his lap belt. The report states that this was permissible and, perhaps surprisingly, did not make any safety recommendations about it.
